Respiratory GP Plus Out of Hospital Services at SALHN

The SALHN Respiratory Out of Hospital Services, based at Noarlunga GP Plus, are a suite of allied health and nursing led programs providing community-based care to patients with chronic respiratory conditions. These programs focus on chronic condition management to support discharge from the acute setting and prevent avoidable hospital readmissions.

The SALHN Respiratory Out of Hospital Services (Noarlunga GP Plus) work in partnership with the FMC inpatient teams and other out of hospital services as well as offering support to the GP and community primary care providers.

The programs are delivered in designated outreach clinics at Noarlunga and Marion GP Plus sites as well as in the patient’s home.

Services provided

Pulmonary Rehabilitation

A centre-based, eight week program of twice weekly education and exercise. The program is delivered by a team of multidisciplinary health professionals (including a GP) to clients with chronic respiratory conditions.

Airway Clearance

Dedicated physiotherapy-led clinics at Noarlunga and Marion GP Plus to assess and treat the airway clearance needs of clients with chronic lung conditions where excess secretions are a feature of their disease and contribute to high infection rates and hospital presentations. This service supports the care of patients with neuromuscular conditions as well as patients with dysfunctional breathing.

Respiratory Nurse Service

Nurse-led clinic at Noarlunga GP+ for patients requiring action plan education and implementation, review of inhaler technique, care co-ordination and general education.

Speech Pathology

Dedicated clinics at Noarlunga and Marion GP Plus for respiratory patients with laryngeal hypersensitivity, vocal cord dysfunction, voice changes, swallowing /aspiration concerns and other speech related assessments and intervention.

Referral information

Please ensure patients meet the referral criteria on the referral form. Forms are to be saved and integrated into relevant GP Practice systems. See fact sheets for further instructions.
