Residential Care Outreach Service at Flinders Medical Centre

The Residential Care Outreach Service provides care for residents of Residential Aged Care Facilities (RACFs) who have presented to hospital, and an outreach service in RACFs to support hospital avoidance or facilitate early discharge. This team is able to provide medical and nursing care, advice regarding management of behaviours of concern, and support with decision making regarding care wishes and palliation where required.

The Flinders Medical Centre Emergency Department based Single Point of Contact (SPoC) Nurse Consultant provides a single point of contact for RACFs, SAAS, and GPs who require support with determining if transportation to hospital is required , and advice regarding alternative care options. Where additional direct nursing support is required, this can be provided by either Hospital at Home (H@H) or the Metropolitan Referral Unit (MRU).

Eligibility criteria

Residential Aged Care Facilities residents who have presented to, or who are at risk of presenting to hospital.

Residential Aged Care Outreach Team (RCOT)

  • Geriatrician-led Medical Team
  • Clinical Nurse Consultant (RICOT Nurse)

Service hours

  • Medical Team: week day medical cover, 8.00am to 5.00pm, available weekend mornings
    Note: AMU Medical Team will provide in-hospital cover outside these hours
  • RCOT Nurse Consultant: 7 days per week, 8.30am to 5.00pm
  • SPoC Nurse Consultant: 7.00am to 9.00pm
