SA Health’s Digital Patient Pathways enables clinicians, nurses, and staff to support a patient’s preparation before their appointment or admission into hospital, as well as after. It gives patients a personalised checklist for:

  • information to provide before or after their appointment or admission,
  • preparation and recovery instructions, and
  • other helpful clinical and administrative information.

Registration and access to the Digital Patient Pathways

SA Health services will invite patients onto applicable digital pathways. The service is free to use and accessible anywhere, anytime on devices with an internet connection via pathways.sahealth.sa.gov.au

Initial registration supports identity verification and consenting to share information with the clinical team or hospital.

Communications from SA Health are via text message (SMS) or email, there is no app required. Text messages that arrive from SA Health include a secure link plus a patient personalised pathway ID.

Partnership

The technology is provided by Personify Care in partnership with SA Health. It is currently used across the Local Health Networks in more than twenty specialities at metropolitan and regional hospitals.
