‘A clear path to care’ is an educational film developed by SA Health for the purpose of training clinical staff about:

  • Advance Care Directives
  • third party consent
  • Resuscitation Plan 7 Step Pathway.

This film is broken into individual chapters and follows a patient’s journey from admission through to discharge illustrating at each key point, best practice for ensuring a patient’s values are known and can be respected when developing clinical instructions for care


General Practice and Advance Care Directives

The part 1 film discusses the relationships between patients, the Resuscitation Plan 7 Step Pathway and Advance Care Directives.


The part 2 film discusses the process for when a patient does not have decision-making capacity and how an Advance Care Directive, a Substitute Decision-Maker “acting in the patient’s shoes” and the Resuscitation Plan 7 Step Pathway can provide guidance in treatment options.


The part 3 film discusses the Resuscitation Plan 7 Step Pathway and requests for Not-for-Transfer to Hospital.


The part 4 film showcases a real-life scenario where Mrs Wells is introduced to Advance Care Directives and Substitute Decision-Makers during an over 75 home health assessment. Later, Mrs Wells visits her GP to provide a certified copy of her completed Advance Care Directive.
