You must be an approved provider to order bulk professionally printed Advance Care Directive Kits.  Approved providers can order Advance Care Directive Kits via the online Advance Care Directives Kit order form.

Quantities supplied are subject to available stock levels. If you place an order in addition to the agreed schedule with the Department for Health and Wellbeing, you may be required to pay a delivery fee.

Becoming an approved provider/collection point

The approved provider / collection point fact sheet (PDF 220KB) outlines the eligibility criteria and administrative arrangements for becoming an ‘approved provider’ and a collection point for members of the public to access free Advance Care Directive (ACD) Kits.
