The Voluntary Assisted Dying Clinical Portal is the secure online system used by medical practitioners to manage requests for voluntary assisted dying in South Australia.
Only authorised medical practitioners and SA Health employees participating in the voluntary assisted dying process can access the Clinical Portal.

People seeking access to voluntary assisted dying can prepare for their first assessment by filling out an optional Prepare for Assessment Form.

It is encouraged that the form is completed on the Voluntary Assisted Dying Clinical Portal.
A login is not required to access the form, however it must be completed in one sitting.
Once completed, a person seeking access to voluntary assisted dying will be given a receipt number to provide to their Coordinating Medical Practitioner. This will automatically update the person’s details in the Clinical Portal when the medical practitioner enters the code at the first assessment.
A hard copy of the form can be downloaded, completed, and brought to the first assessment. The information on the completed form will then be entered into the Clinical Portal by the medical practitioner.

Medical practitioners can only access the Voluntary Assisted Dying Clinical Portal if the Voluntary Assisted Dying Operations Team verifies they are eligible to act as a Coordinating or Consulting Medical Practitioner under the Voluntary Assisted Dying Act 2021 (the Act).

The Voluntary Assisted Dying Operations Team will review your registration, your proof of identity and confirm you are eligible to participate in voluntary assisted dying.
You may be asked to provide more information to confirm your registration or confirm your identity.
If you are eligible, you will get an email telling you if your registration has been successful and a link to access the Voluntary Assisted Dying Mandatory Medical Practitioner Training.

When you have successfully completed the Voluntary Assisted Dying Mandatory Medical Practitioner Training, the Voluntary Assisted Dying Operations Team you will receive an email with a link to activate your Voluntary Assisted Dying Clinical Portal account.

Passwords are active for six months. You will receive an email seven calendar days before your password is due to expire with instructions on creating a new password.
