General Practice Integration Unit (GPIU)

Our General Practice Integration Unit (GPIU) is a dual initiative between the Northern Adelaide Local Health Network (NALHN) and Adelaide Primary Health Networks (APHN).

GPIU was established to drive positive changes and improve patient care by facilitating and strengthening collaboration, communication and integration between General Practices (GPs) in northern metropolitan Adelaide and the NALHN.

The aim of the GPIU is to encourage and improve the exchange of information, cooperation, efficiency, and quality of communication between hospitals and GPs.

Phone calls to our service will be answered between 9:00 am and 4:00 pm, messages can be left outside of these hours and be returned the following business day.

When making contact with the GPIU, please include:

  • your name
  • general practice
  • contact details including an email for follow-up.

The GPIU will endeavour to respond to requests in a timely manner, but a response can take 2-5 business days if there are multiple aspects or multiple patients.

If urgent patient information is required, requests should be directed to the treating team, medical records or outpatients department via the relevant hospital switchboards:

The ED dashboard includes information on the number of patients within our Modbury and LMH EDs, the number of treatment areas available, waiting times, arrivals, discharges and patient types.

Goals/objectives of the unit

  • Connect and facilitate a quality health system and improve your health outcomes.
  • Ensure that GPs are heard, consulted and empowered.
  • Ensure health providers work together.
  • Respond to health needs of the most vulnerable in our community.
  • Improving the referral process, system navigation and the transition of care for patients between primary and hospital care.
  • Engaging and building sustainable relationships between NALHN and general practice to support patient management and continuity of care.
  • The development and implementation of collaborative models of care and pathways.
