Winner  Integrating Care – Closing the Gap on Diabetes

Watto Purrunna Aboriginal Health Service, NALHN

Watto Purrunna (WP) team has initiated a client-centred multidisciplinary team-led diabetes clinic, resulting in clients no longer needing to attend hospital-based clinics. The clinic is conducted in a culturally safe environment, enabling the client access to same day, holistic team-care situated at one site. Clients would otherwise require several appointments over multiple days. Many of the team members are staff who the client is already seeing and familiar with.

Finalist  Tarpari Wellbeing Day

Port Pirie Regional Health Service, CHSALHN

Tarpari Wellbeing Day is a bi annual event held by the Port Pirie Aboriginal Health Team to improve health outcomes for Aboriginal people in the mid north region. 2018 was the most successful event yet with 53 agencies attending to promote health and wellbeing and an approximate 1500 community members attending. The theme for this year’s event was “Looking Forward - New Beginnings”.

Finalist Aboriginal Traditional Healing Services of Ngangkari across NALHN settings

Watto Purrunna Aboriginal Health Service, NALHN

Aboriginal traditional healing services are essential for physical, social, emotional and cultural health and well-being. Innovation from staff to support ongoing and sustainable access to these services at Watto Purrunna primary health care sites, acute and mental health settings in NALHN. The proposed model will provide and support a culturally responsive and respectful health system that contributes to improved health outcomes for Aboriginal people with learnings shared with other LHN’s.
