Country Health Connect offers a wide range of in-home and community-based services to older people, people living with disability, children and carers. Our services will help you or your loved one stay well, active and independent.

Services

A referral is essential to access clinic services.

The Tintinara Country Health Connect clinic does not provide emergency services. Your nearest Emergency Department is located at Meningie and Districts Hospital. Alternatively, you can access the Urgent Care Centre at Keith and District Healthcare, no appointment necessary.

These services are provided by our Registered Nurses (RN) and Enrolled Nurses (ENs) who can provide care to you at our clinics, or in your home.

  • wound care
  • injectable medications
  • post-acute care such as drain management
  • PICC (peripherally inserted central catheter) line care
  • supra pubic catheter care.

Please note that ear syringing, ECG (electrocardiogram), and therapeutic ultrasound services are no longer available at the Tintinara clinic. These services are still available from other local health providers, and transport assistance may be available to some consumers. 

Phlebotomy (blood taking) services continue to be available for consumers who are unable to access a pathology collection centre.

Allied health services offered include:

  • speech pathology
  • physiotherapy
  • occupational therapy.

Non-Aboriginal consumers over the age of 65, and Aboriginal and Torres Strait Islander consumers over the age of 50, also have access to a range of community aged care services.

  • Support at Home Packages
  • Community Home Support Program:
    • Transport (for shopping/pharmacy/medical appointments etc.)
    • Social support, individual or group (this can include shopping or 1:1 assistance with medical appointments)
    • Personal Care
    • Domestic Assistance
    • Meals
    • Mobility Aids.

Community members also have access to a range of specialist services available from visiting nurses.

  • Continence Nurse provides advice, support and management for both children and adults experiencing problems with bladder and/or bowel control.
  • Prostate Cancer Specialist Nurse provides support, information and care from the time of diagnosis. The nurse can be with you and your family throughout the treatment experience including during chemotherapy, radiation therapy and surgery, rehabilitation, counselling, and follow-up survivorship services.
  • Diabetes Nurse Educator provides education and care of diabetes management.
  • McGrath Breast Care Nurse provides support, information and care from the time of diagnosis. The nurse supports you and your family with any treatment, including chemotherapy, radiation therapy and surgery, rehabilitation, counselling and follow-up services.
  • Provision of services to manage lymphoedema
  • Palliative Care Team helps people maintain their quality of life as much as possible while living with a progressive life shortening or terminal illness and uses a multidisciplinary approach to assess and provide services and support to the client and their family.

How to make an appointment

The Coonalpyn clinic is an appointment-only service.

To book an appointment, please contact our Country Health Connect office Monday to Friday, 9:00 am to 5:00pm.

The clinic will be open in accordance with community needs, for example, if there is a clinical need for you to receive injectable medications on a Saturday. Home based services will continue to be provided by appointment. You can nominate to access care in-clinic or at your home.
