Who can access the Clinical Portal?  

Only authorised medical practitioners and SA Health employees participating in the voluntary assisted dying process can access the Clinical Portal.

Medical practitioners can only access the Voluntary Assisted Dying Clinical Portal if the Voluntary Assisted Dying Operations Team verifies they are eligible to act as a Coordinating or Consulting Medical Practitioner under the Voluntary Assisted Dying Act 2021 (the Act). 

SA Health employees participating in the voluntary assisted dying process includes employees of the following services:

  • SA Voluntary Assisted Dying Pharmacy Service (SAVAD-PS)
  • SA Voluntary Assisted Dying Care Navigator Service (SAVAD-CNS)
  • Voluntary Assisted Dying Review Board and Review Board Secretariat
  • Voluntary Assisted Dying Operations Team.

Can patients access the Clinical Portal?  

Patients cannot access or submit a request to access the Clinical Portal.  

All forms required to access voluntary assisted dying must be submitted by the patient’s Coordinating or Consulting Medical Practitioner. 

Patients may choose to complete and submit a Prepare for Assessment Form in the Clinical Portal to prepare for an assessment to access to voluntary assisted dying.

Which forms must be completed through the Clinical Portal?

The following forms must be completed online through the Clinical Portal in line with the Voluntary Assisted Dying Act 2021 (the Act):

  • First Assessment Report Form completed by the Coordinating Medical Practitioner
  • Consulting Assessment Report Form completed by the Consulting Medical Practitioner
  • Final Review Form completed by the Coordinating Medical Practitioner
  • Application for Self-administration Permit completed by the Coordinating Medical Practitioner
  • Application for Practitioner Administration Permit completed by the Coordinating Medical Practitioner
  • Voluntary Assisted Dying Dispensing Form completed by the Dispensing Pharmacist
  • Voluntary Assisted Dying Disposal Form completed by the Dispensing Pharmacist

The following forms are available in the Clinical Portal for the Coordinating Medical Practitioner to download, complete in hard copy and submit a scanned copy in the Clinical Portal in line with the Act: 

  • Written Declaration Form completed by the  person accessing voluntary assisted dying and the Coordinating Medical Practitioner
  • Contact Person Appointment Form completed by the  person accessing voluntary assisted dying
  • Coordinating Practitioner Administration Form completed by the  Coordinating Medical Practitioner

The Clinical Portal also allows medical practitioners to complete the following forms: 

  • Practitioner Registration Form 
  • First Request Form 
  • Consulting Referral Form
  • Transfer of Coordinating Practitioner Role Form
  • Final Request Form.

Can I complete the forms on paper?

All forms must be completed and submitted digitally through the online Clinical Portal. 

In exceptional circumstances, for example, in the case of a system outage, medical practitioners can contact the Voluntary Assisted Dying Operations Team for support to submit a form.

Can I save my form and return to it later?  

Yes, medical practitioners can select to “Save and return later” to save progress and resume filling out the form in future.

How do I know that the SA Voluntary Assisted Dying Review Board has received a form that I have submitted?  

By completing and submitting a form in the Clinical Portal, medical practitioners fulfil their legal obligation to give a copy of that form to the SA Voluntary Assisted Dying Review Board under the Voluntary Assisted Dying Act 2021.

When you submit a form in the Clinical Portal, you will receive an email to your nominated email address confirming that the SA Voluntary Assisted Dying Review Board has received the form.

We recommend you keep a copy of the email for your records. 

You can also view and download any forms you have previously submitted in the Clinical Portal.

How do I know the outcome of an application for a voluntary assisted dying permit?  

You will receive an email to your nominated email address once the outcome of a permit application has been determined.

You will need to log in to the Clinical Portal to view the outcome of the permit application.

Will medical practitioners be able to view other medical practitioners and patients through the Clinical Portal?  

No. The Clinical Portal has been designed to protect the privacy and confidentiality of all users.  

Medical practitioners will only be able to see the details of patients for whom they are a Coordinating or Consulting Medical Practitioner. 

Medical practitioners will only be able to see the details of other medical practitioners if they are a Coordinating or Consulting Medical Practitioner for the same patient. 

How is information in the Clinical Portal kept secure?

A range of security measures are in place to ensure information in the Clinical Portal is kept secure, including:

  • only verified users authorised by the Department for Health and Wellbeing can access the Clinical Portal
  • all users access the Clinical Portal with a unique username and password
  • all users must login to the Clinical Portal using multi-factor authentication
  • users only have permissions to access information that’s reasonably necessary to support a specific patient to access voluntary assisted dying
  • all users are required to take reasonable steps to protect the security of information stored in the Clinical Portal in line with the Voluntary Assisted Dying Clinical Portal Terms of Use (PDF 243KB).

Do I need to install any software? 

No. The Clinical Portal is a secure web-based platform that doesn’t require any software or programs to run.  

You can access the Clinical Portal on any computer device that is connected to the internet.

The Clinical Portal is not compatible with mobile device browsers.

The Clinical Portal is not loading on my computer, what do I do?  

  1. Clear your cache (browser history) and ensure that you only have one browser open at the same time.  
  2. Try another browser, such as Chrome or Firefox. We recommend that you use Chrome.  
  3. Ensure that cookies are enabled in your settings.  
  4. If all else fails, try another computer.
