Geriatric Medicine outpatient services in Central Adelaide Local Health Network (CALHN) are held at the Royal Adelaide Hospital (RAH) and The Queen Elizabeth Hospital (TQEH).

Descriptions of Clinics
General geriatric clinics
These clinics aim to look at the elderly with multiple comorbidities which do not fall into the clinics below. This maybe a combination of frailty, falls, cognitive issues and general medical issues. These clinics occur at:
- RAH — weekly
- Adelaide Hills Community Health Service — fortnightly
- Port Augusta Hospital — monthly
- These are part of the CALHN Memory Service and are weekly. Currently held at the Royal Adelaide Hospital and TQEH
- This operates at Sefton Park Primary Health Care Services or the Queen Elizabeth Hospital fortnightly. See below for access information
Rapid access clinics
- These clinics are set up to enable rapid assessment of patients that need to be seen more urgently.
Stroke (Rehabilitation) follow up clinic
- These clinics are a Multidisciplinary Service including medical aiming at review patients who have had a stroke to be seen 3,6 and/or 12 months after a stroke from inpatient, ambulatory and community.

Please define your referral reason so that it can be triaged to appropriate service.
Falls assessment clinic can be referred directly to the Falls Clinic – see below
All referrals are triaged by the service according to clinical urgency. Patients who are unstable and requiring immediate assessment should be sent directly to the Emergency Department.
To refer your patient, complete and fax a referral to us. For urgent referrals, also contact the Registrar to discuss the case.
To ensure appropriate and timely triage, include all demographic and clinical details as well as relevant investigation results.
If you are concerned about any delay of the appointment or if there is any deterioration in your patient’s condition, contact the Registrar on call on (08) 7074 0000 (RAH).

Community Falls Prevention Service
Falls assessment clinics provide multi-disciplinary assessment and management for older people with complex falls related presentations.
Client consents, is willing to adopt recommended strategies and interventions, and:
- is living in the CALHN region
- is aged 65 years or older, or 50 years for Aboriginal and Torres Strait Islander (ATSI)
- has a history two or more falls in the past 12 months or one fall with a serious injury
- has multiple co-morbidities
- has not had a recent review by a geriatrician or multidisciplinary program
- does not have an acute fracture or acute illness (is medically stable).
Note: permanent residents of high level care are not eligible for this service

Discharge guidelines and information
For those in a residential aged care facility a new referral can be faxed as above and if possible a review in the Residential Aged Care Facility will be done.
As only the GP referral is available to the specialist at the time of triage, simply requesting review without providing information about the problem is insufficient and will lead to significant delays in assessing the referral which will most likely be returned to you for more information.

How to access that clinic
Referral from your GP
You need a referral letter from your GP or medical practitioner to access this service.
Your doctor will need to fax your referral letter to us. We will be in contact with you within clinically recommended times, depending on waiting list length. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time.
