If your patient receives a positive test result, the GP Risk Assessment Teamwill notify you about taking over care of one of your patients.
They will also then call your patient to assess what support they will need during isolation. After this assessment, the GP Risk Assessment Team will be in contact with you to hand over the care of your patient or develop a care plan for their ongoing care throughout their isolation.
