Rapid Detection and Response (RDR) Observation Charts

The purpose of Rapid Detection and Response (RDR) Observation Charts is to assist clinicians to recognise early signs of physiological deterioration, and take appropriate action, which may include triggering a review by other clinicians.

  • Rapid Detection and Response (RDR) Observation charts were introduced throughout SA Health in 2012/2013.
  • The Adult RDR Observation charts were reviewed and updated in 2020.
  • The set of Paediatric RDR Observation charts were reviewed and updated in November 2022.
  • The Neonatal RDR Observation chart – Special Care Nursery/Special Care Baby Unit, was introduced in July 2024.
  • All SA Health RDR Observation charts are available electronically in Sunrise Electronic Medical Record (EMR).
