CALHN Integrated care

Integrated Care (IC) reports to Central Adelaide Local Health Network (CALHN).

We operate a number of services which support patients to avoid unnecessary hospital admissions, as well as access early discharge pathways so you can rest and recover in the comfort of your own home.

Hospital Avoidance and Supported Discharge Service (HASDS)

HASDS focuses on delivering safe, effective, timely and integrated care outside of the hospital setting. This is delivered through a primary care and rapid response model.

We provide flexible, client centred, multidisciplinary clinical intervention. Read more about our services and practitioners:

HASDS — Rapid response clinic (RCC)

HASDS RRC provides a level of care that is outside the clinical capacity of general practice and/or priority care centres, as an alternative to presenting to a CALHN emergency department.

Usual referrers include SA Ambulance Service (SAAS), a patient’s usual GP, and community care providers located in the CALHN catchment area.

The HASDS RRC operates extended hours, 7 days per week (including public holidays).

Patients requiring urgent medical care should continue to be treated through the usual care system (i.e. referred to hospital or triple zero if an emergency).

Outreach Team

Our Outreach Team provides short term care to clients in the community with complex, unmet physical health needs, who are at risk of (re) presenting to a CALHN acute site.

We can provide in-reach to CALHN emergency departments (ED) to assist with ED turn around, as well as support early discharge strategies.

We also provide outreach services to clients in their home, shelters and temporary accommodation, and transient locations where clients are offered linkage to appropriate community services, to improve long term health outcomes.

CALHN Integrated Care Coordinators (CICC)

CICC provide expert clinical advice to external referrers and CALHN clinicians in hospital avoidance strategies and alternate treatment options for patients. We also guide clinicians through appropriate referral processes.
