The option to receive care at home has benefits for both patients and carers.

Home-based care allows for greater involvement of loved ones in the patient’s recovery. Caregivers can provide emotional support, assist with daily tasks and collaborate with healthcare providers to enhance continuity of care.

SA Community Care collaborates with carers while providing care for your loved one at home. The visiting SA Community Care nurse will be able to discuss with you and your loved one the care plan and next steps. As we are a home visit service, we are aware that carers may be involved in the patient’s care as well and we will work together to assist in your loved one’s treatment and recovery. Communication is an important part of the program, please discuss with the nurse or the Metropolitan Referral Unit if you or your loved one need additional assistance or information about their care plan.

Caregivers need care too. We are committed to ensuring you have the support you need while your loved one is accessing SA Community Care and linking you with any additional supports you need as a carer. Carer resources include:

Your loved one’s community service provider will provide contact details, please discuss initial questions or concerns with them.
