Rehabilitation in the Home

Rehabilitation in the Home provides people who have been in hospital with access to rehabilitation therapies and nursing in their home, instead of in a hospital setting. Rehabilitation programs can be provided either face to face during home visits or, when suitable, by using video calls with an iPad which can be provided by the Rehabilitation in the Home service.

Regular therapy sessions are provided in your home at times convenient to you. The rehabilitation team will work with you to develop rehabilitation goals that can be met within the time frame of the program.

There is no cost to access home rehabilitation services.

Services available

Rehabilitation in the Home may include services from allied health staff, nurses and doctors and aims to enable you to regain independence in daily tasks. You may also be provided with equipment on a temporary free loan to support your rehabilitation.

Rehabilitation in the Home provides the following services:

  • rehabilitation and Geriatric Medicine Specialist assessment and review
  • nursing
  • allied Health including:
    • Physiotherapy
    • Occupational Therapy
    • Speech Pathology
    • Dietetics
    • Social Work
    • Exercise Physiology 
    • Allied Health Assistant
  • personal care assistance where this is an identified rehabilitation goal
  • short term equipment loan.

Rehabilitation in the Home services also incorporate access to telerehabilitation services, as described above. 

Eligibility criteria

To access Rehabilitation in the Home, you must:

  • be 18 years or older
  • currently be in hospital and have rehabilitation goals
  • be motivated to participate
  • be medically stable
  • have a home environment that is suitable for therapy.

Accessing Rehabilitation in the Home

Rehabilitation in the Home is provided from several metropolitan hospital locations across South Australia. To access Rehabilitation in the Home, you will need a referral from the team who are providing your care whilst in hospital.
