Descriptions of clinics

Memory clinics

These are part of the CALHN Memory Service and are weekly. Currently held at the RAH and TQEH

Falls clinic

This operates at Sefton Park

Rapid access clinics

These clinics are set up to enable rapid assessment of patients that need to be seen more urgently.

Stroke (Rehabilitation) follow up clinic

These clinics are a Multidisciplinary Service including medical staff.

Patients who have had a stroke are reviewed in clinic 3, 6 and/or 12 months after a stroke.

Referrals are accepted from inpatient teams, ambulatory services and community services. 

General geriatric and aged care clinics

These clinics aim to look at the elderly with multiple co-morbidities which do not fall into the clinics above.  This may be a combination of frailty, falls, cognitive issues and general medical issues. These clinics occur at:

  • RAH weekly
  • Adelaide Hills Community Health Service fortnightly
  • Port Augusta Hospital monthly

GP information and referrals

We accept GP and specialist referrals to all services. 

For urgent referrals, also contact the Registrar to discuss the case. 

All referrals are triaged by the service according to clinical urgency. Patients who are unstable and requiring immediate assessment should be sent directly to the Emergency Department

To ensure appropriate and timely triage, include all demographic and clinical details as well as relevant investigation results. 

If you are concerned about any delay of the appointment or if there is any deterioration in your patient’s condition, contact the Registrar on on-call Geriatrician consultant on call on (08) 7074 0000 (RAH).

Patient information — how to access these services

Referral from your GP

You need a referral letter from your GP or medical practitioner to access all geriatric services.

Once your doctor has referred you, we will be in contact with you to confirm your appointment time and location with the service that best fits your needs.

Appointment times can depend on waiting list length. If there is no waiting list, you will receive an appointment booking letter or we will contact you directly to arrange a suitable time.
