Transition Care Provider and Care Awaiting Placement Panel

The Transition Care Provider (TCP) and Care Awaiting Placement Panel (CAP) is a panel of approved service providers engaged by SA Health to provide Transition Care and Care Awaiting Placement program services. 

Transition Care Program (TCP)

TCP provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay in assisting them to return to their home and avoid being prematurely admitted to residential aged care. 

TCP services are delivered either in residential aged care settings (Residential TCP) or in the individuals own home (Community TCP).

Care Awaiting Placement (CAP)

CAP seeks to enable eligible older people to safely transition from hospital to an interim placement in residential aged care where they receive support to finalise their arrangements for a long-term placement.

CAP is delivered solely in aged care facilities and applies to metropolitan areas only.

CAP is intended to be a short stay program with the length of stay (in most cases) not expected to exceed 21 days.

Without these services, older people may experience inappropriate lengthy stays in a public hospital. 

The TCP / CAP Panel is an essential component of the overall SA Health priority to improve the health and wellbeing of older people and alleviate demand on public hospitals. 

How to join the TCP / CAP Panel

SA Health is taking applications from providers who are interested in joining the panel for the provision of Transition Care and/or Care Awaiting Placement services.

  • Aged Care Quality Standards, or
  • National Safety and Quality Health Service (NSQHS) Standards

Note: Transition Care must comply with the Aged Care Quality Standards, regardless of whether they are accredited under the Aged Care Quality Standards or the National Safety and Quality Health Service (NSQHS) Standards

If you meet the mandated criteria outlined above and are interested in joining the TCP / CAP panel of suppliers, please email: HealthContractNotices@sa.gov.au with TCP / CAP Application Request in the subject field.

Upon receipt of an application request, Invitation to Supply (ITS) documentation will be issued for completion and return.

ITS documentation submitted by a provider will be assessed against a standard set of evaluation criteria. 

Please note that if successfully appointed to the panel, Local Health Networks (LHNs) will be required to conduct a further process to enter into a Customer Agreement between LHNs and a provider for the provision of TCP or CAP services. The Customer Agreement must be executed prior to provision of any services.
